Lawful Presence Verification

Please use this form to submit the following acceptable documents:

A clear copy of an original or certified U.S. Birth Certificate showing the student was born in the U.S. or a U.S. territory. The copy must very clearly show the raised or written seal to be acceptable.
A U.S. Certificate of Naturalization (USCIS form N-550 or N-570).
A U.S. Certificate of Citizenship (USCIS form N-560 or N-561).
A U.S. Certificate of Birth Abroad issued by the Department of State (DS-1350) or a Consular Report of Birth Abroad (FS-240).
Unexpired US passport or the front and back of an unexpired US passport card.
Driver license/State ID cards issued from Georgia and other select states can be accepted under certain conditions: 1. unexpired, 2. the front and back of the card is required, 3. must be a REAL ID (contains a star) not containing any of the verbiage in the chart on our LPV website, 4. no portions of the card may be cut off.
A FN, Limited-Term, Nonrenewable and Nontransferable, Not for REAL ID purposes, T, or Temporary license/ID is not acceptable.
A current military ID (service member only, not dependent). Documented using the Confirmation of Review of Military ID Worksheet – A photocopy is not acceptable
A current, valid Permanent Resident Card (USCIS form I-151 or I-551). We require a clear copy of both the front & back sides of your Permanent Resident Card. It must not expire before the first day of class of the term the student will start KSU.

Only unexpired licenses and state-issued IDs with the REAL ID symbol may be accepted.

Acceptable attachments must be clear and legible. Pictures taken on cellphones, unclear or low-quality attachments may delay processing.

We CAN NOT accept the following documents using this form:

A current military ID (service member only, not dependent).

*Military ID's must be presented in person at the Office of Undergraduate Admissions or Graduate Admissions.
